Palpitation is the uncomfortable awareness of one's own heartbeat. While often benign, it can sometimes signal an underlying arrhythmia. However, palpitations can sometimes signal an electrical problem in the heart. Atrial fibrillation, supraventricular tachycardia, and premature ventricular contractions can all cause palpitations. Palpitations accompanied by fainting, chest pain, or severe shortness of breath require prompt evaluation.
The seriousness of palpitations largely depends on whether underlying heart disease exists. Patients with prior heart attacks, heart failure, or valve disease should take palpitations more seriously.
Evaluation begins with a detailed history and physical examination. The timing, duration, nature, and triggers of palpitations are carefully documented. A 12-lead ECG provides immediate information about the current rhythm.
If palpitations are not daily, a 24-hour Holter monitor is used. For less frequent episodes, an event recorder or 7-14 day extended Holter may be prescribed. Echocardiography assesses heart structure, valve function, and pumping strength. Thyroid function tests, blood count, and electrolyte levels also aid in diagnosis.
Treatment depends on the underlying cause. Benign palpitations often respond to lifestyle modifications (reducing caffeine, stress management, regular sleep). If an arrhythmia is detected, medication therapy or electrophysiological studies with ablation may be recommended.
Chronic arrhythmias like atrial fibrillation may require blood thinners to reduce stroke risk. Beta blockers or calcium channel blockers help control rapid heart rates. All treatment options are individualized based on age, comorbidities, and arrhythmia type.
